Effects of LC+LCBDE+PS in the Treatment of Cholecystolithiasis and Choledocholithiasis

Effects of LC+LCBDE+PS in the Treatment of Cholecystolithiasis and Choledocholithiasis

Abstract

The objective of this study is to determine  the clinical effects of laparoscopic cholecystectomy (LC) combined with common bile duct exploration (LCBDE) and with primary suture (PS) respectively in the treatment of cholecystolithiasis and choledocholithiasis. In our hospital  the surgical treatment of 85 cases of cholecystolithiasis with common bile duct stones were retrospectively analyzed from January 2014 to January 2016, according to the operation method, were divided into 41 cases of group A (LC+LCBDE+PS treatment), group B of 44 cases by endoscopic retrograde cholangiopancreatography/endoscopic sphincterotomy (ERCP/EST) combined with LC therapy. The operation effects of the two groups were compared. No statistically significant differences in operative time of A group and B group (P<0.05); bleeding patients in the A group, hospitalization time, operation cost are lower than those of B group, the differences were statistically significant (P<0.05); One days after operation, two groups of patients with serum IL6, TNFα, CRP, ALT, alpha, AST TBIL  were significantly increased compared with the group before operation(P<0.05), 3 days after operation, two groups of patients with serum IL6, TNF alpha, CRP increased significantly compared with the group before surgery  (P<0.05); after the operation, the rate of complications in 12.20% patients in group A (5/41) and  group B had  no significant difference (10/44) compared to 22.73%(P>0.05). It is found that the effect of LC+LCBDE+PS on cholecystolithiasis combined with common bile duct stones is similar to that of ERCP/EST combined with LC, but it has the characteristics of quicker recovery and lower cost.
